Photoacoustic Image Guided Photodynamic and Immune Combination Therapy based on Exosome for the Treatment of Pancreatic cancer

Exosomes, a natural carriers released by most cells for cell-to-cell communication, have been in the spotlight as drug delivery vehicles in recent studies. In cancer therapy, immunotherapy is also a new technology that uses a patient's innate immune system by stimulating immune cells through antibodies, immune cells and gene regulation. In this study, tumor-derived exosomes were used simultaneously as drug delivery carriers and immunostimulants. Representative sensitizer chlorine e6 was loaded on tumor-derived exosomes upon exosome recombination.during re-assembly of exosomes. Recombinant exosomes have the same membrane protein and average size as exosomes prior to recombination and thus can target tumor cells or tissues. The results showed that recombinant exosomes loaded with chlorine e6 can be visualized by optoacoustic imaging and can efficiently produce reactive oxygen in target cells under laser irradiation. In addition, chlorine e6 loaded recombinant exosomes increased the release of cytokines from immune cells, which means that exosomes can be used as immunotherapy. In conclusion, we developed a strategy to allow photoacoustic imaging-guided photodynamic therapy and immunotherapy for the tumor treatment with tumor-derived recombinant exosomes.
