Little is known about in vivo meniscus and talus cartilage stress in the knee and ankle during downhill walking. The purpose of this study was to develop a knee-ankle joint FE model for predicting menisci and cartilage stress during downhill walking. The three-dimensional finite element model of knee and ankle joint including meniscus, bones, articular cartilage and major ligament was developed form magnetic resonance image. Using the developed model, the meniscus and talus cartilage are investigated at the nine discrete points from 18° to 41° knee flexion angle during downhill walking. Two additional loading cases 100N and 200N are simulated in order to compare the obtained results. The results showed that maximum von Mises stress occurred on the lateral side of meniscus and anterior part of talus cartilage, also the maximum stress of the lateral meniscus increased by 109.5% after applying additional 200N at heel strike. The preliminary results provide some quantitative insight into the effect of additional load in the magnitude of the stress at the knee joint.
